Indoor pyrotechnics must be used with VERY STRICT caution! I've done some small stuff like flash pots and you must be very careful with placement and the amount of material in the cans.
I always test outdoors and pay close attention to height and spread then make sure the charges are WELL under what could create a problem. If there's any question at all that they might impinge on a combustible surface then they don't get used.
When incidents like this one happen it's usually that they went for a "bigger and better" effect and pushed the limits too far.
