I've been using Frontline for years now and my vet splits the large dog size for my cats. The chemical is the same. Metabolism my ass.
I've not had a reaction. I will bet you that people are splitting doses themselves, inaccurately and I'll bet that they're also overdosing small dogs by doubling up on the meds thinking it's going to be a bigger preventative measure in heavy tick areas.
Get your heartworm and flea top spot stuff from your vet and get it accurately measured out.
