This is indeed a can and bottle opener. VERY old but a can and bottle opener nonetheless.
The slot in the center is for pulling small nails.
Many OLD tools would be multi purpose.

The teeth on one side leads me to feel like it is some sort of nut tightener. It kind of reminds me of the adjustable strap wrenches.

I'm sticking with bottle opener. Years ago bottles and cans were made of more durable materials and were thicker and bigger.

OK, we'll go with a bottle opener. Although I want to believe that it would have been sponsored by someone, you know, someone that put a product into bottles, and it is pretty heavy duty for opening bottles.
